The Andrews Sports Medicine Patient Portal, Intellichart, allows you to communicate with your physician’s office directly, refill prescriptions, and pay toward outstanding bills in a secure manner. 

We want to ensure a seamless continuity of care by providing each patient with the ability to communicate with staff directly and view their records online.

Basic Portal Information

Menu: Options on the left side of the screen

Appointments: See upcoming and past appointments for the patient. Request a new appointment.

Messages: This works as a secure message inbox between our clinic and you. If you have a question or a request, this is the place for you. Please, allow a full business day for a response. You can also submit any paperwork that you need us to review.

Forms: This icon will show you the number of forms we currently need from you.  Click the link and see any forms you have pending that need to be completed.  You can also see previously completed forms. You also have access to on-demand forms in the Available Forms section. Prep for your upcoming appointment by printing out and filling out your Medical History and Patient Registration forms. Feel free to attach those completed forms to a Patient Portal Message to Andrews Registration, fax to (205) 314-2559, or bring them with you to your visit.  You can also use any of these forms if you need to update your information since the last time we saw you.

My Chart: Click any of the subheadings under My Chart to see your basic chart information.

OBERD: We use OBERD for our Patient Reported Outcome Measures. These simple surveys allow us to see your improvement based on your view-point. This invaluable data helps us understand your recovery better than ever. The OBERD icon will show you the number surveys you currently have assigned. When you click the link, you will be taken to the OBERD web page to complete your surveys.

Common Q & A

Q: I’m coming for a visit what do I need to do?

A: Check your left side menu. Complete and submit all forms at least 24 hrs prior to your visit. Complete any OBERD surveys. Print out and fill out the Patient Registration and the Medical History forms.  You can attach those completed two forms to a Patient Portal Message to Andrews Registration, fax to (205) 314-2559, or bring them with you to your visit.

Q: What if I have a question for my doctor?

A: On the left side menu select Messages. Compose a new message. Tell us the location, doctor, and topic of your question. In 1000 characters, give us as much detail as possible. If needed, you can send an attachment up to 4mb. Hit “Send Message” when you are done. Allow us 1 full business day for a response.

Q: I want to request an appointment. What do I do? 

A: Select Schedule an Appointment. Fill out the requested information. Please, allow us 1 full business day to review your requests. We will follow-up with you about your requests and let you know available options.

Q: I have a question or request about my records:

A: Send a message to our Records Department.  On the left side menu select Messages. Compose a new message.  Under the “To:” section select “Andrews Records Request.” In 1000 characters, give us as much detail as possible.

Q: I sent a message and I have not heard back. What now?

A: Know that we are working hard to answer you back.  Please, allow us one (1) full business day for a response. We may be able to message you back through the portal. You will receive an email to the email account you attached to the portal letting you know as soon as we message back. We may need to call you for a 1 on 1 conversation to give you the best assistance for your requests.
