Requesting an Appointment

Online: Fill out and submit the appointment request form on our homepage. We will contact you shortly to gather the necessary information and confirm your appointment.

Pre-Registration

A little planning ahead can save you a lot of time in the waiting room. Preregistration will help us move your appointment along as quickly and smoothly as possible on the day of your visit.

First, Download Patient Pre-Registration Packet

You can print, complete and sign them in the comfort of your home.

If it is 48 hours or more before your appointment, you have the option of a full pre-registration. To do this, fax or scan and e-mail the following:

  • Completed Patient Pre-Registration Packet

  • Your insurance card(s) - both primary & secondary (if applicable)

  • Your driver’s license or other government-issued photo ID

If it is less than 24 hours before your appointment, please do not fax or e-mail your registration forms. This will cause delays due to processing time. Instead, bring your insurance card, driver’s license and New Patient forms with you to our office.

If it’s at least two days prior to your appointment: You may fax or e-mail your forms (with cover page) to us. If at all possible, please also scan a legible copy of your insurance card(s) and driver’s license, along with your registration forms. Please, no photos; scans only).

If it’s less than 24 hours before your appointment: To prevent any delay before your visit, please bring registration paperwork with you to your first visit instead of e-mailing or faxing it. Our office needs 24 hours to process registrations that are emailed or faxed.

Note: Some of our physicians may have additional paperwork to be completed at your visit. Please arrive 20 minutes prior to your scheduled appointment in order to complete any additional paperwork before seeing your physician.

What To Bring

If you have not already pre-registered, please complete the Patient Pre-Registration Packet and bring them with you to our office along with:

  • Your insurance card(s) - both primary & secondary (if applicable)

  • Your driver’s license or other government-issued photo ID

If you have seen other doctors and/or been treated for this injury/condition before, please also bring:

  • Any related medical records, studies, or surgery notes (if applicable)

  • Any current or prior imaging (X-ray, MRI, CT) on CD ONLY

  • Written order(s) or referral form(s) from your previous doctor (if applicable)

All patients, whether you have preregistered or not, will need to bring:

  • Any co-payments that may be due

  • A list of any medications you are currently taking, including over-the-counter medicines

Appropriate Attire: You may be asked to change into a gown and/or shorts for your exam. For your comfort, you may choose instead to follow these recommendations. (Be mindful that there could be instances when you may still be asked to wear a gown/shorts.)

  • Knee ExamsWear loose fitting non-denim pants/shorts (no snaps, buttons, zippers or plastic).

  • Hip ExamsWear non-denim shorts, sweatpants, athletic pants, or pull on types (no snaps, buttons, zippers, plastic or metal grommets, embroidery or screen printing).

  • Back ExamsWear non-denim pants and a sports bra (no snaps, buttons, zippers, plastic or metal grommets, screen printing or embroidery).

  • Shoulder ExamsWear a sports bra (no snaps, buttons, or underwire).

Arrival & Check-In

Arrival: Please arrive 20 minutes prior to your scheduled appointment in order to complete any additional paperwork before seeing your physician.

Kiosk Check-In: When you arrive, you may be directed to one of our self-check-in kiosks (at our Birmingham location only). Staff members will greet you and assist you with kiosk check-in.

Questions For Your Physician

In addition to examining you, your doctor will talk to you about your injury or condition. You can help your doctor diagnose and treat you by being prepared to answer these common questions:

  • What are your symptoms?

  • When did these symptoms start? What were you doing when they started?

  • What activities make your symptoms worse/better?

  • Have you had this problem before? If so, what was the diagnosis and how was it treated?

  • Have you ever had surgery in that area?

  • What home treatments have you tried, including nonprescription medications? Did they help?

Appointment Policies
Late Arrival

Please arrive for your scheduled appointment on time. We recommend that new patients arrive 20 minutes early to fill out any necessary paperwork. Patients who arrive more than 30 minutes after the beginning of their scheduled appointment time may need be rescheduled, and may be charged a late cancellation fee.

Cancellation Policy

24-hour notice is required for cancelled appointments. A $25 cancellation fee will be imposed for any office appointment not cancelled with a 24-hour notice. Repeatedly not showing up for your scheduled appointments may result in discharge from the practice.

Appointment Duration & Wait Time

You should allow a minimum of two hours for your visit. The first 30 to 45 minutes of your appointment will be for handling check-in, insurance, paperwork and X-rays.

We are a high-volume practice. Our physicians are committed to spending adequate time with every patient to listen to their situation, properly diagnose their condition, discuss recommended forms of treatment, and address any additional questions and/or concerns they may have. Please be aware that this may result in long wait times.

Insurance Information
Accepted Insurance Plans

Our participation with insurance plans is regularly reviewed and changed. We recommend that you contact your insurance carrier directly to verify your coverage prior to making an appointment. You will be responsible for any expenses not covered by your insurance.

Please Note: Provider participation in an insurance plan is at the physician level, not the group level. So while most Andrews Sports Medicine physicians participate with the following plans, some may not. Please call our office with any questions.

The following list includes the major insurance plans with which Andrews Sports Medicine currently participates:

  • Blue Cross Blue Shield Alabama Plans

  • Blue Cross Blue Shield Alabama Medicare Advantage Plans

  • Standard Medicare Part B

  • Railroad Medicare

  • VivaHealth (Note: Viva has certain closed-panel plans that restrict patient access to specific providers. Call Viva to check your plan.

  • VivaHealth Medicare (Call Viva to verify your plan's status with us.)

  • Tricare (Standard, Prime, Extra and For-Life)

  • Aetna (PPO Networks)

  • Cigna (PPO Networks)

  • United Healthcare (PPO Networks)

  • Health Choice (Alabama)

If you do not find your plan listed, please call our office or your insurance carrier to see if we are a participating provider with your plan.
Submitting a Claim to Insurance

For patients who have insurance from one of our participating insurance carriers, Andrews Sports Medicine will submit all claims and necessary supporting documentation on your behalf.

Deductibles & Co-Payments

Most insurance policies today include provisions for out-of-pocket deductibles and co-payments. A deductible is a pre-determined amount of money for which you are responsible before your insurance benefits kick in. A co-payment is a flat rate you are expected to pay up-front for specific services, and does not count toward your deductible.

The amount of your deductible and/or co-payment will vary based on your plan. Any co-payment required by your plan will be collected at the time of your visit. Payments may be made by cash, personal check (no greater than $200), VISA, MasterCard or Discover Card.

Non-Covered Services

Our office offers some forms of treatment that are not currently covered by insurance plans. If these services are recommended for you, you will be notified of ALL out-of-pocket expenses prior to receiving the treatment.  You will be required to sign a “non-covered service waiver,” and payment will be due in full prior to receiving the treatment. 

Some of our most common non-covered services include plasma-rich plasma (PRP) therapy and bone marrow aspiration therapy.

Referrals, Worker’s Comp, MVA & Litigation
Referrals

If a referral is required by your insurance carrier, it is your responsibility to get the referral and provide it to our practice. If you do not have a referral in place when you arrive for your visit, you may be held responsible for full payment for your office visit or procedure, or your appointment may need to be re-scheduled.

Worker’s Compensation

All workers’ compensation appointments MUST be scheduled by a workers’ compensation adjuster or case manager, as our practice CAN NOT make initial appointment directly with patient.

Andrews Sports Medicine will submit "open" claims only on your behalf, unless we are aware in advance that the claim will be denied. It is the patient's responsibility to provide the office staff with complete insurance information, as well as your case management contact and phone number.

Andrews Sports Medicine will bill your primary health insurance in cases where worker's compensation denies the claim for any reason. Any balance not paid by the worker's compensation carrier or primary health plan will be the patient's responsibility.

Motor Vehicle Accident (MVA) & Litigation Cases

Andrews Sports Medicine does not recognize MVA or litigation claims. As such, you will be classified as a self-pay patient unless you have active health insurance coverage and we receive a letter of subrogation from your insurance company, in which case we will submit all claims to your health insurance carrier.

We do not need a letter of subrogation from patients covered by Blue Cross Blue Shield AL.

If your commercial health insurance carrier then determines that the claim should be refunded due to third party liability, you will be responsible for the bill to handle through a litigation attorney. Liability action against someone else is not a reason to delay payment. Payment of the bill is the responsibility of the individual who has received the treatment, not the individual who is being sued.

Fees & Financial Information
Self-Pay Policy

New patients who do not have health insurance coverage are considered “self pay.” As such, you must pay a deposit of $300.00 up front at the time of the visit. Payment must be made in cash, by credit card or money order (personal checks will not be accepted). Subsequent visits or surgery will require additional pre-payment. You will not be seen until payment is made.

Out-of-Pocket Expenses

Out-of-pocket expenses will depend upon your insurance coverage. After we’ve received final payment from your insurance company, you will be billed for any remaining patient responsibility balance, as specified by your carrier. If deductibles have not been met, you are responsible for those.

Out-of-Network Patients

If your insurer is not contracted with Andrews Sports Medicine, you will be considered an out-of-network patient. Andrews Sports Medicine will, as a courtesy to you, submit your claim to your insurance carrier. However, your out-of-pocket expenses may be higher than those you would otherwise incur with a provider who participates with your insurance plan.

Please note: For those insurance carriers that Andrews Sports Medicine does not participate with, the claim check may be mailed directly to you. In these cases, you agree to and are responsible for signing and forwarding the check to our billing office.

Hospital Bills & Physician Fees

If you receive services in the hospital from your primary care physician or from other specialists such as anesthesiologists, assistant surgeons or consultants, you or your insurance carrier will be billed separately by them. If you have any questions about these bills, please contact the billing department at the hospital or surgical center where you had the procedure. Andrews Sports Medicine has no control over these charges.

The charges for your stay are based on how long you are in the hospital and the services you receive during that time. There is a basic room charge, which includes fees for the room, nursing care, laundry, housekeeping and other services. There are additional charges for any other services you receive, including but not limited to: operating and recovery room charges, laboratory tests, radiology, physical therapy, medications, and special supplies. These charges are itemized on your bill from the hospital.

Cancellation Policy

24-hour notice is required for cancelled appointments. A $25 cancellation fee will be imposed for any office appointment not cancelled with a 24-hour notice. Repeatedly not showing up for your scheduled appointments may result in discharge from the practice.

Returned Checks

A fee of $30.00 will be charged to your account for any/all returned check(s). All future payments must then be made by cash, charge, money order or bank check.

Prescription Refills
Policies

No medications will be prescribed if you are not a current patient who has been treated within the last 6 months. If you would like a long-term anti-inflammatory or any other type of medication, we suggest you discuss this with your primary care physician.

Many narcotic pain medications CANNOT be called in. It is our office policy NOT to mail these prescriptions. If you require a refill or change of narcotic medication, you or a designee must pick it up at our office. Please bring a driver’s license when picking up a prescription from our office.

No prescriptions will be called in:

  • After 4:00 PM (CST) Monday–Thursday

  • After 12:00 PM (CST) on Friday

  • On weekends

Refill By Phone

Call your pharmacy to renew your prescription. They will contact us for any necessary information.

Please allow at least 24-48 hours for your prescription to be renewed, as each request has to be approved by a physician and not all physicians are in the office on any particular day.
Helpful Tips

If your prescription is running low, please do not wait until the weekend or until your medication has completely run out. If you are planning to travel out of town, please make sure you have enough medication to last for your whole trip.

Requirements
  • Ages 18 or older MUST SIGN THE FORM.
     
  • Under 18, Legal guardian must sign for the patient.
     
  • Form must be filled out completely. Provide an accurate fax number or an accurate address.
Charges/Fees For Medical Records
  • There is no charge for written medical reports.
  • If you need an X-Ray CD it is a $5.00 charge.

    (a) Must be paid BEFORE we order X-Ray CD

    (b) X-Ray CD must be mailed; therefore, we need an accurate address on the release form that indicates where to send your records.
Disability, FMLA & Claim Forms
  • It is an approximately 7 to 10 BUSINESS day wait on ALL forms to be completed by the doctor.
     
  • ALL forms must be paid for BEFORE we can send them.
     
  • There is a $15.00 charge for a one-page form, EVERY other form is $25.00
     
  • Payment can be made by mail, paying online yourself at the "Pay Your Bill" icon, bringing the payment to Andrews Sports Medicine, or we can take payments over the phone.
